Service Description
Our Check-Ins and Ongoing Support meetings are designed to provide participants, families and support networks with a dedicated space to review current supports, discuss any changes in circumstances, update goals and ensure services continue to meet individual needs. These meetings help keep supports aligned, effective and person-centred, while also offering an opportunity to raise concerns, celebrate progress and plan for future supports. We aim to provide flexible, open communication in a supportive environment where participants feel heard, understood and involved in decisions about their care and supports.
